Transition Services
Students and their families work closely with teachers and CES vocational transition specialists to design and implement individual transition plans. Transition planning must begin "not later that the first I.E.P. to be in effect when the child turns 16, or younger if determined appropriate.
Transition planning looks at a student's strengths, interests, and preferences to create a plan for a student's future. Transition goals are developed as part of student's IEP through the process of a Planning Placement Team (PPT) meeting. Transition planning areas include:
- Employment
- Post-Secondary Education
- Independent Living
- Building a Bridge

Purpose
- Individual and group counseling related to individual goals and PPT recommendations
- Provide classroom staff with intervention and prevention strategies.
- Conduct clinical assessment of student functioning as deemed appropriate.
- Ongoing consultation with classroom staff, LEA contacts, community based agencies, and families.
- Participation in PPT meetings, and behavioral support meetings.
- Maintain confidentiality of student information, performance, and records.
- Collaboration with CES personnel and consultants.
- Complies with all Federal, State, and agency regulations on conduct and ethical behavior.
